Please complete a referral form in as much detail as possible. Once completed, send the form to: [email protected]
Your form will be sent to your midwife and she will contact you to arrange a booking appointment between 8 to 10 weeks of pregnancy.
The midwife holds a session on a Thursday and Friday Afternoons.
After Birth
Your surgery will contact you to arrange your mother and Baby 6-8 weeks antenatal check with your doctor. Please ensure you bring your child’s red book.
