Brief Summary:

The facility to access documents via Patient Access is currently unavailable due to technical problems.  We are working with the relevent parties to resolve this issue and hope to have it fixed shortly.

 

Registering for Patient Access allows you to book appointments and gain access to your medical records online.

Step One

To register for this service you will need to download and complete the form below and email or bring into reception. You will then be sent, via email, your login details. Please check your spam folder if you have not received an email by 5 working days.

Step Two

Once you have received your email with login details, you can register on the Patient Access website. You will then have full access to the on line booking system and access your records.

Step Three

f you require specific medical information, you will need to complete a Subject Access Request Form (SAR). Once completed the form will be authorised by your doctor and the information will be collected. This process can take up to 1 month.
